Consent/Photo Release Form

Your task is to create a Consent/Photo Release Form that governs the usage of the photographs taken at events where subjects are not directly contracting with the photographer, such as at public events or in classes. This form ensures that you have the necessary permissions to use the photographs taken in these scenarios.

To create a comprehensive Consent/Photo Release Form, use the following details:
Full Name or Company Name of the Photographer: "{field_1_input}"
Names of the Subjects (or description of the group): "{field_2_input}"
Description of the Event or Setting: "{field_3_input}"
Specific Terms of Consent: "{field_4_input}"
Rights Granted by Subjects to Use Their Images: "{field_5_input}"
Limitations or Restrictions on the Use of Photos, if any: "{field_6_input}"
Date of Consent: "{field_7_input}"

Ensure that you create a detailed Consent/Photo Release Form that clearly highlights the rights of using images from subjects who are not directly contracting with the photographer.
